Upload your documents
- There are three (3) documents that need to be uploaded
  - Proof of program completion – please verify program eligibility directly with the program you completed
  - NBHWC Coaching Log
  - NBHWC Education Attestation Form OR Work Experience Document
- Please view the “How To” videos for step by step instructions on uploading documents

*Application fees are non-refundable (NBHWC Fee Policy). Please verify you meet all eligibility requirements prior to applying.
*NBHWC is committed to providing test accommodations to examinees with documented disabilities under the Americans with Disabilities Act (ADA) that would effect their ability to take this exam.
